
While the system won't be identical to that of Diablo II, it will be very similar. Items will drop randomly and have random attributes. Some of the things that definitely changed:

Diablo III expands the player character's arsenal by leaps and bounds. Many items make a return in the form of magic, rare and unique forms. It has been mentioned that some of the more popular Diablo I and II items may make a return, but they will be far less powerful than the new Legendary breed of items, the new name of unique items.
The item name colors have undergone a change, though normal and magic items are still white and turquoise respectively, there are purple colored items as seen in the gameplay video. The items also have a visual background color in the inventory depending on their type. Items with magical attributes will also give a visual effect when equipped by the character. This is a great improvement from the different color palettes that were used in Diablo II's magic items.
Elixirs provide effects for a limited period, and have a number on the end indicating the rank of the elixir (so far, only rank 1 elixirs were seen). Higher ranks of Elixirs could mean better effects and/or longer lasting time. (Standard lasting time is 300 seconds as was seen in the BlizzCon Demo).
Scrolls are small, stackable, 1-time-use items that provide effects for a limited period. World of Warcraft has similar scrolls, which buff the player for 30 to 60 minutes with various buffs (mostly stats and armor). They too have ranks, except they are represent by an adjective instead of a number, that could mean better effects and/or longer lasting time.
Amulets are worn in the Amulet Slot. There are various designs of amulets, but their effects/stats are determined by what item type they are, since each item type has its own rules regarding effects/stats.
Rings are worn in the Rings Slots. There are various designs of rings, but their effects/stats are determined by what item type they are, since each item type has its own rules regarding effects/stats.
Gems are small, precious stones that are magically enchanted and can be socketed into an item, to give it a permanent effect (it is not known whether you can replace socketed gems). Each gem type has properties that give a specific effect depending where you socket it. Ranks of gems exist, and are named by how fine they are cut (Chipped, Flawless). As stated in a recent talk with Jay Wilson, Sockets are random and affect all items, meaning that 1 player may get a Legendary item while another player gets that same item but with a socket(s).
Note: "..." = the name of the item. So ".... of Storms" could mean "Heavy Axe of Storms" or "Short Sword of Storms"

This is probably the most lovely lore source feature for players who decide to immerse themselves into the world of Sanctuary. Let’s say you enter a dungeon, and find a pedestal. When you highlight the pedestal, by moving your mouse cursor over it and clicking it, you loot the book. Open your Inventory (I) and click the book or scroll. A sound clip pops up at the bottom-right of the screen. Underneath the clip are three buttons: play, stop, close. It is a voice over recording telling the story within the book.
For example, in the Cathedral of Tristram, the Journal of King Leoric tells his story during the time Diablo possessed his son Albrecht (Diablo I).
